Repeat Prescriptions

Repeat prescriptions may be requested by:

1. Using the stub from your previous repeat prescription to mark the repeat medication required and send to reception or
2. Faxing your prescription request to the surgery or
3. E-mailing the request to 3VMP.prescriptions@dudley.nhs.uk or
4. Online via the form on this page

You will need to include your FULL name and date of birth. Please list for EACH item you require - medication/product name, type (tablets/syrup/cream, etc), strength, dose, code, etc.

NOTE: You should clearly indicate whether you wish to collect the prescription from either Stourbridge or Wollaston surgeries or if you prefer, it can be sent to Boots at the Stourbridge Health Centre or at Wollaston. If you do not indicate a preference, the prescription will be available for collection from the Stourbridge surgery.

Please allow 48 hours for the doctor to check and sign your prescription.
If you wish your prescription to be posted please provide a stamped addressed envelope. If you wish to use a chemist’s services, you must make these arrangements yourself with the chemist of your choice.

We do not accept repeat prescription requests by telephone.

Prescriptions Charges and Exemptions

Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs).

The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines.  Prescription prepayment certificates (PPCs) offer real savings for people who need extensive medication.
